Operator error blamed for fatal crash of medical helicopter in New York

In a tragic incident that occurred in April 2022, a Mercy Flight helicopter crashed during a training exercise in Elba, New York, resulting in the deaths of the pilot and flight instructor. The National Transportation Safety Board (NTSB) recently released their final report, which indicated that operator error was the likely cause of the crash.

According to the report issued on December 8th, investigators highlighted "inappropriate flight control inputs" by the pilots while practicing for an unstable condition known as vortex ring state. These errors caused the main rotor blade to make contact with the helicopter's tail, ultimately leading to the mid-flight break-up. The NTSB also noted that the flight instructor's "inadequate monitoring of the flight" contributed to the accident.

The pilot, James Sauer, hailing from Churchville and a retired New York state police pilot, tragically lost his life in the crash. The flight instructor, Stewart Dietrick, from Prosper, Texas, also died in the incident. Both individuals were highly experienced and well-regarded in the aviation community.

The ill-fated Mercy Flight helicopter was on its second training run of the day when it crashed in a field in Elba. Fortunately, there were no patients on board at the time of the accident. The helicopter was a Bell 429, a twin-engine aircraft utilized by Mercy Flight to transport patients to hospitals in the Buffalo area, according to the not-for-profit company's website. The aircraft was typically housed at the Genesee County Airport, which is located less than 3 miles from the crash site.

The NTSB report shed light on the events leading up to the crash, based on interviews and available data. The report revealed that during the earlier training flight of the day, the pilot was surprised by the lack of intervention from the flight instructor. After being instructed to perform a vortex ring state recovery maneuver, the helicopter experienced an unexpectedly high descent rate. The pilot mentioned feeling uncomfortable and expressed concern about the instructor's passive behavior, stating that the instructor had his hands on his lap. Consequently, the pilot took it upon himself to exit the dangerous descent rate without further guidance from the instructor.

Following the release of the report, there was no immediate response from Bell Helicopter, the manufacturer of the aircraft, based in Fort Worth, Texas. It is unclear whether they will provide any comment or response regarding the NTSB's findings.

The Mercy Flight crash has left the aviation community in shock and mourning. Investigations like this play a crucial role in understanding the causes of accidents and implementing safety measures to prevent similar incidents in the future. The loss of two skilled and dedicated professionals serves as a reminder of the risks inherent in the field of aviation and the need for constant vigilance.
