According to a preliminary report released by the National Transportation Safety Board (NTSB), a dead goose was discovered in the flight control system of the helicopter. The helicopter, operated by Air Evac Lifeteam, was en route back to Weatherford after transporting a patient to an Oklahoma City hospital when the accident took place. The victims of the crash included the pilot, as well as a flight nurse and a paramedic who were part of the Air Evac Lifeteam crew. The NTSB report states that the aircraft was located in a pasture near Hydro, approximately 60 miles west of Oklahoma City.

The report confirms that all major components of the helicopter were found at the accident site, with the fuselage, engine, and tail rotor forming the main part of the wreckage. The transmission, however, had separated from the rest of the wreck. Additionally, a motor blade was discovered in an adjacent field, while the mast nut was found within the debris field.
